Consumer Health Data Privacy Policy
WHY THIS POLICY EXISTS:
Washington's My Health My Data Act protects health information that falls outside traditional medical-records law, including information you share with a health care practice before you become a client. This policy describes how Northwest Relationships handles that information, called consumer health data, and the rights you have over it. Once you become our client, your records are protected by HIPAA and Washington's health care information laws and governed by our Notice of Privacy Practices.
CONSUMER HEALTH DATA WE COLLECT:
We collect only what you choose to share with us and what is needed to operate our website:
● Information in your inquiries: your name, contact information, and anything you tell us about the care you are seeking (for example, the type of therapy you are interested in), when you submit a form, call, or email us.
● Technical website data: IP address, browser and device information, and pages visited, collected by our website platform to operate and secure the site.
We collect this information directly from you and from your use of our website. We do not collect consumer health data from other sources, do not use advertising trackers or pixels, and do not infer health information about you from other data.
HOW WE USE IT:
We use consumer health data only to respond to you, answer your questions, schedule care, operate and secure our website, and meet our legal obligations. These are the purposes for which you provide it, and we do not use it beyond them.
SHARING, AND WHAT WE NEVER DO:
We share consumer health data only with service providers that process it on our behalf to deliver what you have asked for, our website platform (Wix) and, when you move toward becoming a client, our scheduling and electronic health record systems, all bound to confidentiality, or when the law requires disclosure.
● We do not sell consumer health data, and we never will.
● We do not share consumer health data with third parties for advertising or marketing.
● We do not use advertising pixels, conversion tags, or analytics that send your data to ad platforms.
● We do not use geofencing around any health care facility.
YOUR RIGHTS:
Under the My Health My Data Act, you have the right to:
● Know what consumer health data we have collected about you, and receive a list of the third parties and affiliates with whom we have shared it, with contact information.
● Withdraw any consent you have given for collection or sharing.
● Have your consumer health data deleted, including from our service providers and backups as the law provides.
HOW TO EXERCISE THEM:
Email info@northwestrelationships.com or write to Northwest Relationships, Attn: Privacy Officer, 5702 N 26th Street, Suite B, Tacoma, WA 98407. We will confirm your request, verify your identity, and respond within the timeframes the Act sets. If we decline a request, we will explain why, and you may appeal by replying to our response; we will answer your appeal in writing. If you are not satisfied, you may contact the Washington Attorney General's Office at www.atg.wa.gov/file-complaint.
We will never deny you services, charge you differently, or treat you differently for exercising these rights.
CHANGES:
If our practices change, we will update this policy before the change takes effect and note the new effective date here.
